Abstract: This paper provides an evidence-based review of the pathophysiology and diagnosis of irritable bowel and leaky gut syndromes. Available conventional (pharmaceuticals and psychological interventions) as well as complementary and alternative treatments (hypnotherapy, herbal medicines and probiotics) for irritable bowel syndrome are discussed. Potential approaches to the treatment of leaky gut (e.g. normalization of intestinal permeability by avoidance of NSAIDs and ethanol, reducing intestinal damage from endotoxins by using antioxidants, and using bowel lavage with saline solution, anti-lipopolysaccharide antibodies and methylprednisone to address endotoxaemia) are also identified.
